The T cell receptor (TCR) recognizing cockroach allergen-derived peptides presented on MHC class II is a specialized protein complex on the surface of CD4+ T cells that mediates the immune response to cockroach antigens. It specifically binds to peptide fragments from major allergens such as Bla g 1, Bla g 2, Bla g 5, and Bla g 9 when they are displayed by Major Histocompatibility Complex (MHC) class II molecules (HLA-DR, DQ, or DP) on antigen-presenting cells (Oseroff et al., 2012). This recognition event is the primary driver of allergic sensitization and the subsequent Th2-polarized inflammatory response characterized by the production of cytokines like IL-4, IL-5, and IL-13 (da Silva Antunes et al., 2021). These cytokines promote the synthesis of allergen-specific IgE and the recruitment of eosinophils, leading to the clinical manifestations of asthma and allergic rhinitis, particularly in urban environments (Schulten et al., 2019). Therapeutically, this TCR-MHC-peptide interaction is targeted by allergen-specific immunotherapy (AIT), which uses cockroach extracts to induce immunological tolerance (da Silva Antunes et al., 2025). Experimental approaches also include peptide immunotherapy (PIT) using specific T cell epitopes, such as those derived from Per a 5, to induce T cell anergy or regulatory T cell (Treg) expansion (Sharma et al., 2022). Unlike whole-extract therapies, peptide-based modulators are designed to target the TCR without cross-linking IgE on mast cells, thereby reducing the risk of systemic allergic reactions like anaphylaxis.
Allergen-specific immunotherapy (AIT) and peptide immunotherapy (PIT) modulate the TCR-mediated signal to induce immunological tolerance through T cell anergy, induction of regulatory T cells (Tregs), or immune deviation from a Th2 to a Th1-type response (da Silva Antunes et al., 2025; Sharma et al., 2022).
