Overview

Collagen alpha-1(IV) chain (COL4A1) is a protein encoded by the COL4A1 gene that is a major component of type IV collagen, a key structural element of basement membranes throughout the body[1][3][6][8]. This protein combines with other collagen IV chains to form a covalently cross-linked, flexible network critical for mechanically supporting tissues, shaping organs, filtering blood (notably in kidney glomeruli), and facilitating key biological processes such as cell adhesion, migration, differentiation, and survival[2][4][8]. COL4A1 is especially important for the integrity of vascular basement membranes. Mutations in COL4A1 are implicated in several hereditary diseases characterized by fragile blood vessels, such as COL4A1-related brain small-vessel disease, porencephaly, hereditary angiopathy with nephropathy, aneurysms, and muscle cramps, with involvement described in the brain, eyes, kidneys, muscles, and cardiovascular system[1][3]. COL4A1 is not currently considered a conventional therapeutic target like a receptor, enzyme, ion channel, or transporter, and there are no approved drugs that specifically and directly target this protein. However, its biomarker status is increasingly recognized for patient selection and risk stratification in genetic vascular disorders, particularly those involving small vessels and basement membrane integrity[1][3].
