Overview

High-Intensity Focused Ultrasound (HIFU) stimulates collagen synthesis by delivering ultrasound energy deep into the dermis and subcutaneous tissue, causing controlled thermal coagulation. This process activates dermal fibroblasts, leading to increased synthesis of collagen (types I and III) and elastin, which results in skin firming and rejuvenation. The cellular mechanism involves decreased Caveolin-1, activation of ERK1/2 and p38 MAPK signaling, reduced p53 activity, and enhanced cell cycle progression. HIFU is widely used in aesthetic medicine for skin tightening and wrinkle reduction. It is not a molecular target itself, but refers to an induced biological response involving multiple molecular factors in the skin[1][4][6][7][9].

Mechanism of action

HIFU delivers focused ultrasound energy to the dermis and subcutaneous layers, generating thermal and mechanical effects. - This triggers a wound-healing response: - Heat stimulates fibroblasts, inducing proliferation and migration[6][8]. - Fibroblasts increase production of collagen (primarily types I and III) and elastin[1][4][7][9]. - Cellular signaling is modulated: Caveolin-1 (Cav-1) levels decrease, ERK1/2 and p38 MAPK pathways are activated, p53 activity is reduced, and cell cycle progression is promoted[1][6][7]. - Remodeling phase involves conversion of type III to type I collagen with tissue tightening[9].
