Collagen type IV alpha 3 chain (COL4A3) mRNA is the transcript that encodes the alpha 3 subunit of type IV collagen, a fundamental structural component of the alpha 3-4-5(IV) heterotrimer, which forms the scaffold of the glomerular basement membrane (GBM) in the kidney, as well as basement membranes in the inner ear and eye [1.2.1, 1.3.2]. In healthy individuals, this collagen network maintains the integrity of the glomerular filtration barrier and supports renal function [1.1.1, 1.2.2]. Mutations in the COL4A3 gene lead to Alport syndrome, a hereditary disorder characterized by progressive renal failure, sensorineural hearing loss, and ocular abnormalities [1.2.1, 1.2.2]. Additionally, the COL4A3 protein is the target of autoantibodies in Goodpasture syndrome, leading to severe glomerulonephritis and pulmonary hemorrhage [1.2.1, 1.2.3]. As a therapeutic target, COL4A3 mRNA is being investigated for mRNA replacement therapy, where functional transcripts are delivered via lipid nanoparticles (LNPs) to podocytes [1.2.5, 1.3.1]. This approach aims to restore the production of functional collagen IV trimers, thereby repairing the GBM and slowing the progression toward end-stage renal disease [1.3.2, 1.3.3]. Current research focuses on optimizing delivery systems to penetrate the glomerular filtration barrier and achieve sustained protein expression [1.3.2]. Challenges in targeting this molecule include the need for precise delivery to podocytes and the management of potential immune responses to exogenous RNA or delivery vehicles [1.3.2].
