Overview

Collateral dredging (often translated from the Chinese term 'Tongluo') is a therapeutic concept and systemic mechanism of action primarily utilized in Traditional Chinese Medicine (TCM) and integrative pharmacology. It refers to the restoration of microvascular function and the clearance of physiological obstructions within the 'collaterals,' a biological network roughly analogous to the microcirculatory system and capillary beds (PubMed Central, 2025). Pharmacologically, collateral-dredging therapies are investigated for their ability to improve blood flow, stabilize atherosclerotic plaques, and promote angiogenesis in hypoxic tissues (Journal of Traditional Chinese Medicine, 2023). This effect is typically achieved through the use of multi-component herbal formulas that modulate diverse molecular targets, including VEGF for vascular repair and MAPK for inflammatory control (Frontiers in Pharmacology, 2024). Because it describes a holistic physiological process and a therapeutic methodology rather than a specific protein, enzyme, or receptor, it is categorized as a therapeutic mechanism rather than a discrete molecular target.

Mechanism of action

Collateral dredging operates through a synergistic, multi-pathway mechanism that includes the activation of the VEGF/VEGFR2 pathway to promote angiogenesis, inhibition of the p38 MAPK and NF-κB pathways to reduce inflammation, and regulation of the PI3K/AKT/mTOR pathway to induce autophagy and protect against cell apoptosis in ischemic or hypoxic environments.
