Overview

Colonic water absorption and luminal osmotic balance represent the physiological mechanisms responsible for regulating fluid volume and solute concentration within the large intestine (StatPearls, 2023). This process is primarily driven by the active transport of electrolytes, such as sodium and chloride, which creates osmotic gradients that water follows passively via paracellular and transcellular pathways, including water-selective channels like aquaporins (PubMed, 2001). Key molecular players in this balance include the sodium-hydrogen exchanger 3 (NHE3), epithelial sodium channels (ENaC), and the cystic fibrosis transmembrane conductance regulator (CFTR) (NIH, 2016). Pathological alterations in these pathways can result in clinical conditions such as malabsorptive diarrhea, where water remains in the lumen, or chronic constipation, resulting from excessive fluid absorption. Therapeutic strategies often involve osmotic laxatives to retain water in the lumen or secretagogues that stimulate ion secretion to increase stool moisture and facilitate transit. Maintaining this balance is essential for systemic fluid homeostasis and effective waste elimination.
