The combination of metformin and agave inulin is a therapeutic strategy designed to manage metabolic disorders by pairing a first-line pharmacological agent with a prebiotic fiber. Metformin primarily lowers blood glucose levels by reducing hepatic glucose production and increasing peripheral glucose uptake through the activation of AMPK (PubChem). Agave inulin, a highly branched fructan, serves as a substrate for beneficial gut bacteria, promoting a healthy microbiome and the production of short-chain fatty acids (Frontiers in Immunology, 2021). These metabolites further support metabolic health by stimulating the secretion of incretin hormones like GLP-1, which regulate insulin and appetite. Research indicates that this combination may offer synergistic benefits, such as improved glycemic control and enhanced weight loss, compared to metformin monotherapy (Journal of Functional Foods, 2019). By targeting both systemic metabolic pathways and the gut-brain-liver axis, this combination addresses the multifactorial nature of type 2 diabetes and obesity.
Metformin acts by inhibiting mitochondrial complex I and activating AMP-activated protein kinase (AMPK), which suppresses hepatic gluconeogenesis and improves insulin sensitivity (StatPearls, 2023). Agave inulin acts as a prebiotic fiber that is fermented by colonic bacteria into short-chain fatty acids (SCFAs) like butyrate and propionate. These SCFAs bind to G protein-coupled receptors (GPR41/43) on intestinal L-cells to stimulate the release of glucagon-like peptide-1 (GLP-1), which enhances insulin secretion and promotes satiety (PubMed, PMID: 31614458).
