Commensal and less-susceptible host-associated microorganisms refer to the vast and diverse community of non-pathogenic bacteria, fungi, and viruses that reside on or within the human body, particularly in the gastrointestinal tract. These microorganisms are essential for maintaining host health by performing metabolic functions such as the synthesis of vitamins K and B12 and the fermentation of dietary fibers into beneficial short-chain fatty acids (NIH Human Microbiome Project). They also play a pivotal role in 'colonization resistance,' where they prevent the establishment of pathogens by competing for resources and modulating the host's immune system (Nature Reviews Microbiology, 2013). In the context of drug development, this group is typically viewed as a 'non-target' or 'off-target' population that should be spared during antimicrobial therapy to prevent dysbiosis. Broad-spectrum antibiotics often deplete these beneficial populations, leading to complications such as Clostridioides difficile overgrowth, whereas narrow-spectrum or microbiome-sparing drugs like fidaxomicin are designed to leave these commensals intact (Clinical Infectious Diseases, 2012). Consequently, preserving the integrity of these microorganisms is a major focus in modern therapeutic strategies to avoid secondary infections and maintain systemic homeostasis.
