Conjunctival mast cells are resident immune cells in the ocular surface tissues that serve as primary effectors in allergic eye diseases [1]. They are characterized by the presence of cytoplasmic granules containing inflammatory mediators such as histamine, proteases (tryptase and chymase), and cytokines [2]. Upon activation, primarily through the cross-linking of IgE bound to high-affinity FcεRI receptors by specific allergens, these cells undergo a complex process of degranulation [3]. This release of mediators leads to vasodilation, increased vascular permeability, and recruitment of other inflammatory cells, resulting in the clinical manifestations of allergic conjunctivitis, such as pruritus and edema [4]. Pharmacological management involves mast cell stabilizers and dual-acting agents that prevent the release of these pro-inflammatory substances, thereby providing symptomatic relief and preventing the late-phase allergic response [1][5].
