Consciousness is an emergent physiological state characterized by arousal (wakefulness) and awareness (the content of experience), resulting from complex interactions within the central nervous system, particularly the thalamocortical system [1, 16]. Although not a single molecular target, receptor, or enzyme, it represents a primary clinical and therapeutic endpoint for pharmacological interventions in anesthesiology and psychiatry [4, 6]. The neurobiological basis of consciousness involves high-level integration of information across diverse brain regions, typically explained through frameworks such as the Global Neuronal Workspace Theory or Integrated Information Theory [7, 16]. Pharmacological modulation is primarily achieved by drugs that target ion channels and neurotransmitter receptors, such as GABA_A receptors for sedation, NMDA receptors for dissociation, or 5-HT2A receptors for altered states of awareness [4, 10, 18, 19]. Pathological alterations, known as Disorders of Consciousness (DoC), result from severe structural or metabolic brain injury and include conditions such as coma, vegetative states, and minimally conscious states [1, 4].
Pharmacological modulation of consciousness is achieved through various molecular pathways that regulate neural integration and arousal, most notably the potentiation of inhibitory GABA_A receptors, the antagonism of excitatory NMDA-type glutamate receptors, and the agonism of cortical 5-HT2A serotonin receptors.
