Core 2 O-glycan antigen is a structural motif in mucin-type O-glycoproteins, consisting of a GlcNAcβ1-6GalNAc linkage attached to serine or threonine residues on proteins. It acts as an extended branching structure built upon Core 1, predominantly found in tissues with high O-glycosylation (intestine, immune cells). Core 2 O-glycans play essential roles in mediating cell-cell interactions, regulating immune responses through selectin ligand formation, and influencing protein stability and trafficking. They are implicated in cancer and inflammation, as changes in Core 2 structure and density can alter immune cell behavior and malignant potential. Additionally, Core 2 O-glycans serve as scaffolds for further carbohydrate modification, such as addition of sialyl-Lewis x, creating functionally important oligosaccharide epitopes. The term "Core 2 O-glycan antigen" therefore refers to a structural glycan epitope rather than a classical drug target or receptor.
