Overview

The corneal epithelium serves as a multi-layered, stratified squamous epithelial physical barrier that protects the eye from environmental, chemical, and biological insults. Its protection is conferred by various cell junction proteins—tight junctions, adherens junctions, desmosomes, hemidesmosomes, and gap junctions—located in distinct epithelial layers. The apical (outermost) layer's tight junctions are the primary resistance barrier, sealing intercellular spaces to prevent paracellular ingress of pathogens and allergens. The basement membrane further contributes by restricting microbial invasion and supporting epithelial stability. The tear film covering the cornea also lubricates the surface, promoting comfort, washing away irritants, and preventing desiccation. Damage or dysfunction of this barrier is implicated in dry eye, allergies, infection, and other ocular surface diseases.
