Overview

The corneal stromal collagen network is the primary structural framework of the eye's cornea, accounting for approximately 90% of its total thickness and providing the mechanical strength necessary to maintain ocular shape and intraocular pressure (StatPearls: Cornea Anatomy, 2023). It is composed of highly organized Type I and Type V collagen fibrils arranged in parallel lamellae, a configuration that is essential for optical transparency and refractive power (PubMed: PMID 25703474). In pathological conditions such as keratoconus or post-LASIK ectasia, the enzymatic degradation or mechanical weakening of this network leads to progressive corneal thinning and visual distortion (NIH/NEI: Keratoconus, 2022). This network serves as a therapeutic target for corneal collagen cross-linking (CXL), a procedure that utilizes riboflavin as a photosensitizer and UV-A light to create new covalent bonds between collagen fibers (American Academy of Ophthalmology: Corneal Cross-linking, 2023). By increasing the biomechanical stiffness of the stroma, CXL effectively halts the progression of ectatic diseases and prevents the need for corneal transplantation. Additionally, the network is a focus for regenerative medicine approaches aiming to restore corneal clarity after scarring or infection (Nature Communications: PMID 30135450).
