Corneal stromal hydration state refers to the physiological balance of water within the corneal stroma, which is essential for maintaining corneal transparency and refractive power. In a healthy eye, the stroma is kept in a state of relative dehydration, known as deturgescence, typically around 78% water content, through a "pump-leak" mechanism. The "leak" is driven by the swelling pressure of stromal proteoglycans, such as lumican and keratocan, which naturally imbibe water, while the "pump" is maintained by the corneal endothelium through active ion transport involving Na+/K+-ATPase and carbonic anhydrase. Disruption of this state, often due to endothelial dysfunction or surgical trauma, leads to corneal edema, resulting in light scattering and significant vision loss. While not a single molecular target, it is a critical therapeutic endpoint managed through osmotic agents like hypertonic saline to draw out excess fluid or through drugs like Rho kinase inhibitors that aim to improve the functional capacity of the endothelial pump. Additionally, "stromal hydration" is a clinical technique used during cataract surgery to seal corneal incisions by inducing localized swelling.
