Overview

The corneal surface and tear film represent a dynamic physiological system essential for ocular homeostasis and optical quality. The tear film is a complex mixture of lipids, water, and mucins that coats the corneal and conjunctival epithelia, providing a smooth refractive interface and protecting the eye from environmental insults (StatPearls, 2023). Its biological roles include lubrication, antimicrobial defense, and the delivery of oxygen to the avascular cornea (NIH, 2022). Pathological disruption of this system, often termed Dry Eye Disease (DED), involves tear film instability and hyperosmolarity, leading to chronic inflammation and potential damage to the corneal epithelium (PubMed, 2021). Pharmacological treatments target this system by supplementing tear volume with lubricants, stabilizing the lipid layer, or reducing ocular surface inflammation through immunomodulators like cyclosporine (Journal of Clinical Medicine, 2020). This functional unit is critical for maintaining the transparency and health of the cornea, which is the eye's primary refractive element.
