The corpus cavernosum is one of a pair of sponge-like regions of erectile tissue that contain the majority of blood in the penis during an erection (StatPearls, 2023). It is an anatomical structure rather than a specific molecular target; however, it contains various molecular targets such as phosphodiesterase type 5 (PDE5) and alpha-adrenergic receptors that are critical for pharmacological intervention (NIH, 2022). In the flaccid state, the smooth muscle of the corpus cavernosum is tonically contracted; upon sexual stimulation, the release of nitric oxide leads to the production of cyclic guanosine monophosphate (cGMP), causing muscle relaxation and blood engorgement (PubMed, 2021). Dysfunction in these signaling pathways leads to conditions like erectile dysfunction, which is commonly treated with PDE5 inhibitors that facilitate blood flow within this tissue (Wikipedia, 2024). Other clinical pathologies associated with this tissue include priapism, a prolonged and painful erection, and Peyronie's disease, which involves the development of fibrous scar tissue within the corpora (Mayo Clinic, 2023).
Drugs typically target specific molecular components within this tissue, such as Phosphodiesterase type 5 (PDE5) to inhibit cGMP degradation or Prostaglandin E1 receptors to stimulate adenylate cyclase, both resulting in smooth muscle relaxation and increased blood inflow (StatPearls, 2023).
