The corpus cavernosum is one of a pair of sponge-like regions of erectile tissue, which contain most of the blood in the penis during an erection (StatPearls, 2023). It consists of irregular blood-filled spaces called sinusoids, which are lined by endothelial cells and surrounded by smooth muscle and a fibroelastic extracellular matrix (Wikipedia, 2023). Physiologically, the corpus cavernosum facilitates penile tumescence through the relaxation of trabecular smooth muscle in response to nitric oxide, a process mediated by the secondary messenger cyclic guanosine monophosphate (cGMP) (PubMed, 2021). While not a single protein or receptor, it serves as the functional site for various pharmacological interventions, most notably phosphodiesterase type 5 (PDE5) inhibitors like sildenafil, which prevent the breakdown of cGMP to maintain blood flow (NIH, 2022). Pathological conditions associated with this tissue include erectile dysfunction, priapism (persistent and painful erection), and Peyronie's disease, which involves the development of fibrous scar tissue (Mayo Clinic, 2023). Therapeutic challenges include the risk of priapism, a medical emergency where blood becomes trapped in the cavernous spaces, potentially leading to permanent tissue damage (StatPearls, 2023).
Pharmacological agents typically target enzymes (e.g., PDE5) or receptors (e.g., EP receptors) within the smooth muscle cells of the corpus cavernosum to increase cyclic guanosine monophosphate (cGMP) or cyclic adenosine monophosphate (cAMP) levels, leading to smooth muscle relaxation and increased blood inflow (NIH, 2022; PubMed, 2021).
