Overview

This entry collectively refers to three distinct costimulatory molecules: CD40, CD80, and CD86, which are critical for immune regulation but are not a single entity. CD40 is a member of the tumor necrosis factor receptor superfamily, expressed primarily on antigen-presenting cells, important for B cell activation and differentiation through CD40L engagement. CD80 and CD86 are both B7 family members, expressed on antigen-presenting cells, providing essential costimulatory signals for T cell activation by binding to CD28 or CTLA-4 on T cells. Although functionally similar, they are distinct. All three are validated therapeutic targets in areas such as cancer, autoimmunity, and transplantation. Their expression levels can serve as biomarkers, and therapeutic interventions carry safety concerns related to immune dysregulation, including cytokine release syndrome or infection risk.

Mechanism of action

These molecules play distinct roles in immune regulation. CD40 mechanism involves antibody-mediated agonism or antagonism of CD40 and blocking CD40–CD40L interactions. CD80 and CD86 mechanisms primarily involve inhibition of their interactions with CD28, blocking T cell costimulation and modulating immune checkpoints.
