It integrates sensory inputs from vagal afferents detecting airway irritants and coordinates the sequential motor phases of coughing—inspiration, compression, and expiration—via efferent signals to respiratory muscles through the vagus, phrenic, and spinal nerves. This network shares elements with the respiratory rhythm generator, reconfiguring during cough to produce forceful expulsion while adapting from normal breathing patterns. In disease, hypersensitivity in this circuit contributes to chronic cough and airway protection failures, as seen in asthma or post-viral conditions. Antitussive drugs target multiple sites within this network, suppressing cough but risking diminished airway clearance. Recent studies highlight roles for additional nuclei like SP5C in cough initiation, emphasizing its multifunctional brainstem basis.
