The cAMP signaling pathway in pancreatic beta cells is a critical regulatory network that modulates glucose-stimulated insulin secretion (GSIS). Upon activation of G protein-coupled receptors such as the Glucagon-like peptide-1 receptor (GLP-1R) or the Glucose-dependent insulinotropic polypeptide receptor (GIPR), adenylyl cyclase converts ATP into cAMP. This secondary messenger acts through two primary effectors: Protein Kinase A (PKA) and Exchange Protein Directly Activated by cAMP 2 (Epac2/RAPGEF4). These effectors facilitate the closure of K(ATP) channels, increase intracellular calcium, and directly promote the fusion of insulin vesicles with the plasma membrane. Beyond acute secretion, cAMP signaling supports beta-cell health by promoting proliferation and inhibiting apoptosis, making it a central focus for Type 2 diabetes therapies. Drugs targeting this pathway, particularly GLP-1 receptor agonists and DPP-4 inhibitors, are widely used to improve glycemic control by enhancing the incretin effect.
Activation of G protein-coupled receptors (e.g., GLP-1R, GIPR) leads to the stimulation of adenylyl cyclase, increasing intracellular cAMP levels. This cAMP then activates Protein Kinase A (PKA) and Exchange Protein Directly Activated by cAMP 2 (Epac2), which collectively enhance the exocytosis of insulin-containing granules and promote beta-cell survival.
