Overview

Cyclic nucleotide-gated channel subunit beta 1 (CNGB1) is a transmembrane protein that serves as a critical component of the cyclic nucleotide-gated (CNG) ion channels in rod photoreceptors (UniProt Q14028). These channels are essential for the phototransduction cascade, where they mediate the influx of sodium and calcium ions in response to light-induced changes in cyclic GMP (cGMP) levels (NCBI Gene 1258). CNGB1 acts as a modulatory subunit, ensuring the proper assembly, trafficking, and sensitivity of the channel complex (PMID: 29202463). Mutations in the CNGB1 gene are a known cause of autosomal recessive retinitis pigmentosa 45 (RP45), a condition characterized by the progressive degeneration of rod photoreceptors followed by cone loss (PMID: 34050318). This disease typically manifests as early-onset night blindness and gradual constriction of the visual field, eventually leading to severe vision impairment. Currently, CNGB1 is a primary target for gene augmentation therapies, such as AAV5-CNGB1, which utilize adeno-associated virus vectors to deliver a functional copy of the gene to the retina (NIH/NCATS). These therapies aim to restore normal channel function and slow or halt the progression of retinal degeneration in patients with biallelic CNGB1 mutations. Clinical evaluation of these treatments often involves monitoring biomarkers such as electroretinography (ERG) responses and retinal structure via optical coherence tomography (OCT).
