Cytochrome P450 21-hydroxylase (CYP21A2) is a microsomal enzyme crucial for adrenal steroidogenesis, catalyzing the 21-hydroxylation of progesterone and 17α-hydroxyprogesterone to produce cortisol and aldosterone. Mutations in CYP21A2 cause congenital adrenal hyperplasia (CAH), leading to impaired cortisol and aldosterone synthesis and excess androgen production.
