Cytochrome P450 11B1 is a mitochondrial enzyme catalyzing the final step in cortisol and corticosterone biosynthesis. It hydroxylates steroids at the C-11β position. Mutations lead to congenital adrenal hyperplasia due to 11-beta-hydroxylase deficiency, causing impaired cortisol synthesis, accumulation of mineralocorticoid precursors, and excess androgen production.
