Overview

Cytokines are a diverse group of small signaling proteins, including interleukins, interferons, and chemokines, that act as critical mediators of the immune system and inflammatory responses (StatPearls: NBK545261). The cytokine and cytokine production pathways refer to the integrated network of intracellular signaling cascades, such as the NF-κB and JAK/STAT pathways, that govern the synthesis, secretion, and cellular response to these molecules (Nature Reviews Immunology, 2018, 18:373-388). These pathways are essential for coordinating the body's defense against pathogens and facilitating tissue repair; however, their chronic or excessive activation is a hallmark of numerous pathologies, including rheumatoid arthritis, psoriasis, and inflammatory bowel disease (NIH: PMC2785020). In severe cases, such as sepsis or COVID-19, an uncontrolled "cytokine storm" can lead to systemic organ failure and death (PubMed: 32645083). Pharmacological targeting of these pathways has revolutionized the treatment of autoimmune disorders through the use of monoclonal antibodies that neutralize specific cytokines or small molecules that inhibit downstream signaling enzymes (Frontiers in Pharmacology, 2021, 12:675834). Despite their efficacy, these therapies require careful monitoring due to the risk of profound immunosuppression and the potential for opportunistic infections (Journal of Clinical Medicine, 2021, 10:2832).
