Overview

Cytomegalovirus (CMV) and Epstein-Barr virus (EBV) peptide-Human Leukocyte Antigen (HLA) complexes are cell-surface molecular assemblies that present viral protein fragments to the immune system (Nature Reviews Immunology, 2002). These complexes are formed when viral antigens, such as CMV pp65 or EBV LMP1/2, are processed intracellularly and loaded onto HLA class I or II molecules for transport to the plasma membrane (Journal of Virology, 2014). They function as the essential ligand for T-cell receptors (TCRs) on CD8+ and CD4+ T cells, facilitating the recognition and destruction of infected cells (PubMed: 15620463). In clinical contexts, these complexes are critical targets for treating opportunistic infections and virus-associated malignancies, such as post-transplant lymphoproliferative disorder (PTLD), in immunocompromised patients (Blood, 2017). Therapeutic interventions include virus-specific T-cell (VST) therapies, such as Tabelecleucel (Ebvallo), which utilize the specificity of TCRs to bind these pHLA complexes and induce targeted cytotoxicity (EMA, 2022). Monitoring HLA compatibility and viral load is vital for the efficacy and safety of these treatments to prevent complications like graft-versus-host disease (Lancet Oncology, 2022).
