Cytomegalovirus pp65 peptide-MHC complexes are critical immunological targets formed by the presentation of peptides derived from the UL83 (pp65) tegument protein on the surface of host cells via Major Histocompatibility Complex (MHC) molecules. The pp65 protein is the primary internal matrix protein of Human Cytomegalovirus (HCMV) and serves as the dominant target for the host's cellular immune response, particularly CD8+ cytotoxic T lymphocytes (CTLs). In healthy individuals, these complexes are recognized by the immune system to maintain viral latency; however, in immunocompromised patients, such as those undergoing hematopoietic stem cell or solid organ transplantation, the failure of this recognition leads to life-threatening CMV reactivation. Therapeutic strategies targeting these complexes include adoptive T-cell therapies (ACT), where pp65-specific T cells are infused into patients to restore antiviral immunity, and TCR-like antibodies or vaccines designed to elicit a robust CTL response. The most well-characterized version of this complex involves the NLVPMVATV peptide presented by the HLA-A*02:01 allele, which is a frequent focus of clinical diagnostic and therapeutic development.
