Debridement is a fundamental clinical procedure in wound management that involves the removal of necrotic, damaged, or infected tissue to promote healing and minimize the risk of infection. It is not a biological molecule, receptor, or transporter, but rather a therapeutic process essential for treating chronic wounds, severe burns, and pressure ulcers. The procedure can be performed through various modalities, including surgical (sharp), mechanical, autolytic, and enzymatic methods. In the context of pharmacology, enzymatic debridement utilizes specific protein-degrading agents, such as Collagenase clostridium histolyticum or bromelain-derived enzymes (e.g., NexoBrid), to selectively hydrolyze structural components of the necrotic eschar. By clearing non-viable material, debridement reduces bacterial bioburden and inflammatory signaling, thereby facilitating the transition of a wound from a stagnant inflammatory state to a proliferative healing phase.

Mechanism of action

Enzymatic debridement involves the application of proteolytic enzymes to a wound bed to selectively or non-selectively hydrolyze peptide bonds in proteins such as collagen, fibrin, and elastin found in necrotic tissue, thereby facilitating the separation of the eschar from healthy underlying tissue.
