Overview

Decreased hepatic glucose production refers to the reduction of glucose output by the liver, a key target in managing hyperglycemia, especially in type 2 diabetes. The liver produces glucose primarily through two processes: gluconeogenesis (synthesis of new glucose from non-carbohydrate sources) and glycogenolysis (breakdown of stored glycogen). Modulating these pathways is central to glycemic control. Insulin suppresses hepatic glucose production, while glucagon and catecholamines stimulate it. Pharmacological agents like metformin reduce liver glucose output by affecting mitochondrial respiration, cAMP levels, and expression of key gluconeogenic enzymes. Targeting decreased hepatic glucose production is a cornerstone strategy for treating type 2 diabetes.
