Demineralized enamel matrix refers to tooth enamel that has undergone loss of mineral content—primarily calcium and phosphate ions—from the hydroxyapatite crystal matrix, usually as a result of a local acidic environment (pH < 5.5).[1][4] Enamel itself is the hardest substance in the body, composed of approximately 95–98% mineral (hydroxyapatite), with minimal organic content.[1][4][7] When demineralization occurs, the enamel's crystal structure shrinks, pore volume increases, and its structural integrity is compromised, leading to the formation of carious lesions if not reversed.[1][4] Demineralized enamel is not a discrete biopolymer, receptor, or protein but a state of the enamel tissue reflecting a pathology relevant to dentistry, particularly in caries development and prevention.[1][4]
Remineralizing agents (e.g., fluoride) promote reuptake of minerals into demineralized enamel. Sealants and other dental treatments protect or restore the demineralized enamel surface.
