A demineralized enamel subsurface lesion, commonly known as a white spot lesion, is an early stage of dental caries characterized by the loss of mineral content beneath a relatively intact surface layer of enamel (StatPearls, 2023). This condition arises when the local pH drops due to bacterial acid production, causing the dissolution of hydroxyapatite crystals and increasing the porosity of the enamel structure (Journal of Dentistry, 2019). Although it is a clinical pathology rather than a specific molecular receptor, it serves as the primary site of action for remineralization therapies (NIH, 2021). Treatments such as fluoride applications and calcium-phosphate delivery systems work by diffusing into the subsurface pores to facilitate the precipitation of new mineral phases, such as fluorapatite (PubMed, 2020). Successful management of these lesions can arrest or reverse the decay process, preventing the need for invasive restorative procedures (Cochrane Database, 2013). The lesion's optical properties change due to increased porosity, resulting in the characteristic opaque white appearance that serves as a diagnostic marker. Monitoring these lesions often involves advanced imaging techniques like Quantitative Light-induced Fluorescence to assess mineral density changes over time. Therapeutic success is measured by the reduction in lesion size and the restoration of enamel translucency and hardness.
