Overview

Demodex mites, primarily Demodex folliculorum and Demodex brevis, are microscopic arachnids that reside within human hair follicles and sebaceous glands (StatPearls, 2023). While they are part of the normal skin microbiome, an overgrowth of these mites is linked to inflammatory disorders such as rosacea and blepharitis (NCBI, 2020). The mites contribute to disease through mechanical blockage of glands, direct tissue damage, and the release of internal bacteria like Bacillus oleronius, which triggers a host immune response upon the mite's death (Journal of Medical Microbiology, 2007). Pharmacological intervention typically targets the mite's nervous system using acaricides like ivermectin or lotilaner to reduce mite density (FDA, 2023). Additionally, antibiotics may be used to mitigate the inflammatory effects of the bacteria released by the mites. Managing the mite population and the subsequent inflammatory cascade is the primary goal in treating Demodex-related pathologies.

Mechanism of action

Acaricides like ivermectin and lotilaner target the mite's nervous system by binding to glutamate-gated or GABA-gated chloride channels, leading to paralysis and death (FDA, 2023). Antibiotics like metronidazole target the associated bacteria by inhibiting nucleic acid synthesis (PubMed, 2020).
