Overview

Dengue virus non-structural protein 1 (NS1) is a multifunctional viral glycoprotein, 46–55 kDa in size, that exists intracellularly as a dimer associated with membranes and as a secreted hexamer in the extracellular space. NS1 is essential for dengue virus (DENV) RNA replication, acting as part of the viral replication complex, and directly modulates infectious particle assembly by interacting with structural viral proteins (Envelope, precursor Membrane, and Capsid). It is a key pathogenic factor contributing to dengue disease severity: secreted NS1 in blood disrupts endothelial cell barriers, induces vascular leakage similar to septic shock, and stimulates cytokine release from immune cells via Toll-like receptor 4 (TLR4) and complement activation, driving inflammatory responses. NS1 binds and alters high- and low-density lipoproteins (HDL, LDL) and interacts with host serum proteins influencing lipid metabolism. NS1 is a major target for diagnostic ELISA tests due to its high abundance in serum during acute DENV infection, and its levels are used as early markers and predictors of severe disease progression. It is also the focus of vaccine research—anti-NS1 monoclonal antibodies can protect against lethal dengue challenge in preclinical models, although cross-reactive and pathogenic antibody responses remain a challenge. Some experimental peptide inhibitors interfere with NS1 function to suppress viral replication. Overall, Dengue virus non-structural protein 1 is a validated and essential therapeutic target for diagnostic, preventive, and potentially antiviral strategies against dengue disease, with extensive roles spanning viral replication, immune modulation, and severe disease pathogenesis.
