Overview

Dengue virus nonstructural protein 5 (NS5) is the largest and most conserved protein of the dengue virus (DENV), serving as a critical multi-functional enzyme for viral replication and host immune evasion [1, 4]. It contains two major functional domains: an N-terminal methyltransferase (MTase) domain responsible for RNA capping and a C-terminal RNA-dependent RNA polymerase (RdRp) domain that catalyzes the synthesis of the viral RNA genome [1, 5]. The MTase domain performs N7 and 2'-O methylation, which is essential for viral mRNA stability and for shielding the virus from host innate immune detection [3, 6]. Additionally, NS5 acts as a potent antagonist of the host's interferon response by mediating the proteasomal degradation of STAT2, thereby facilitating viral spread [9, 14]. As a therapeutic target, NS5 is highly attractive because its RdRp activity is unique to the virus and lacks a direct human homolog, offering a pathway for selective antiviral design [4, 5]. Current drug development efforts focus on nucleoside analogs that cause premature RNA chain termination and non-nucleoside inhibitors that target allosteric pockets to disrupt polymerase activity [2, 11]. Other strategies involve inhibiting the MTase domain or disrupting the essential interaction between NS5 and the viral helicase NS3 [8, 12]. Despite its potential, challenges such as ensuring pan-serotype efficacy and avoiding off-target toxicity have hindered the clinical approval of NS5-targeted therapies to date [1, 13].

Mechanism of action

Inhibition of RNA-dependent RNA polymerase (RdRp) activity through nucleoside analog chain termination or allosteric modulation; inhibition of methyltransferase (MTase) activity to block RNA capping; disruption of NS3-NS5 protein-protein interactions; and inhibition of NS5 nuclear import.
