Guanine nucleotide exchange factor (GEF), DENN domain-containing protein, Rab GTPase regulator, Vesicle trafficking protein, Other (contains RUN and PLAT domains, related to membrane interactions and cytoskeletal dynamics)
01
Overview
DENN domain-containing protein 5B (DENND5B) is a large (~145 kDa) guanine nucleotide exchange factor (GEF) that activates Rab GTPases, orchestrating GDP-GTP exchange to regulate vesicular trafficking, particularly in chylomicron secretion from the Golgi to the plasma membrane. It contains canonical DENN domains, two RUN domains (involved with cytoskeletal dynamics and Rab regulation), and a PLAT domain (associated with membrane interactions). DENND5B is highly expressed in the brain, small intestine, and liver, and is essential for lipid absorption and neural development. Functionally, it interacts with MYO5A (myosin-Va) and valosin-containing protein (VCP), supporting actin-based secretory vesicle transport and protein quality control. Genetic variation in DENND5B influences metabolic traits such as plasma lipids and body mass index, and aberrant expression is linked to cancer and neurodevelopmental disorders. It is increasingly recognized as a potential prognostic biomarker and an emerging molecular target in metabolic and oncological contexts.

Mechanism of action
Not applicable as there are no approved drugs or advanced clinical candidates with defined mechanisms acting on DENND5B.
03
Biological functions
Positive regulation of membrane trafficking and vesicle-mediated transportGuanyl-nucleotide exchange (GDP-GTP exchange for Rab GTPases)Chylomicron secretion and triglyceride transportNeural development and brain-specific expression, especially in early embryogenesisCoordination of intracellular lipid processing and absorptionInteraction with cytoskeletal motor proteins (e.g., myosin-Va) for vesicle transport
04
Disease associations
Cancer (prognostic biomarker for colorectal cancer; linked to aberrant gene expression and poor survival)Lipid metabolism disorders (mutations linked to altered plasma lipid profiles, obesity resistance, and altered body composition)Neurodevelopmental disorders (potential involvement in brain malformation based on paralog studies; associated with Lissencephaly 8 and Epilepsy)Cardiovascular disease (through influence on plasma lipids and atherosclerosis in knockout models)Other (possible links to Wilms’ tumor via ceRNA networks; exploratory relationships)
05
Safety considerations
Loss-of-function or knockout causes profound changes in metabolism, including resistance to obesity and altered lipid absorption; this implies risk for adverse effects on nutritional status and homeostasis if targetedPotential consequences for neural development if disrupted in pediatric applicationsUnknown off-target effects due to broad tissue expression (liver, intestine, brain, etc.)
06
Biomarkers
High DENND5B mRNA expression as a prognostic biomarker for colorectal cancer and possibly other malignanciesIntronic variant linked to cancer risk loci (colorectal cancer)Potential for monitoring chylomicron secretion/lipid metabolism in metabolic syndrome and cardiovascular risk assessment
