Dental nerves refer to the sensory nerve fibers, primarily from the maxillary and mandibular branches of the trigeminal nerve, that innervate the dental pulp and periodontal tissues (StatPearls, NBK482283). These nerves consist of A-delta and C-fibers that transmit nociceptive signals in response to thermal, mechanical, or chemical stimuli (PubMed, 28292395). While not a single molecular target, they are the anatomical site for local anesthetic action, where drugs like lidocaine block voltage-gated sodium channels to prevent pain transmission (StatPearls, NBK532905). Inflammation of these nerves, known as pulpitis, is a major cause of acute dental pain and often requires endodontic or pharmacological management (PubMed, 30123932). Understanding the distribution and physiology of these nerves is critical for effective regional anesthesia and the treatment of dentin hypersensitivity (StatPearls, NBK482283). These nerves also play a role in proprioception and the regulation of blood flow within the dental pulp (PubMed, 28292395).
Local anesthetics bind to the alpha-subunit of voltage-gated sodium channels on the internal surface of the nerve membrane, preventing the influx of sodium ions and the subsequent depolarization required for action potential propagation (StatPearls, NBK532905).
