Dental stains refer to the discoloration of the tooth structure, categorized primarily as extrinsic or intrinsic based on the location and source of the pigment. Extrinsic stains are located on the tooth surface or within the acquired pellicle, typically resulting from the accumulation of chromogens from food, beverages (e.g., coffee, tea), tobacco, or chromogenic bacteria. Intrinsic stains occur when chromogenic materials are incorporated into the enamel or dentin, often due to systemic factors such as tetracycline use during tooth development, excessive fluoride intake (fluorosis), or pulpal necrosis. While dental stains are not biological receptors or enzymes, they are clinical targets for aesthetic treatments involving chemical oxidation. Bleaching agents, such as hydrogen peroxide or carbamide peroxide, penetrate the tooth structure to release free radicals that oxidize and break down the double bonds in organic chromogen molecules, resulting in a lighter appearance.
