Overview

Dentin hydroxyapatite and dentinal tubule surfaces are the fundamental structural targets in restorative dentistry and the management of dentin hypersensitivity (StatPearls, NBK560571). Dentin is a mineralized connective tissue consisting of a hydroxyapatite-rich matrix [Ca10(PO4)6(OH)2] permeated by thousands of microscopic dentinal tubules that house odontoblastic processes and interstitial fluid (PubChem, CID 14781). According to the hydrodynamic theory, the exposure of these tubules due to enamel loss or gingival recession allows external stimuli to cause fluid shifts, which activate intradental nerves and result in pain (PubMed, 11014151). Pharmacological and biomaterial interventions target these surfaces to either occlude the tubule openings or reinforce the mineralized matrix through remineralization. Common agents include fluorides, bioactive glasses (e.g., NovaMin), and oxalates, which form insoluble precipitates that seal the tubules and reduce dentin permeability (Journal of Dentistry, 2013). Understanding the interaction between these agents and the dentin surface is crucial for developing effective treatments for tooth sensitivity and preventing the progression of dental caries.

Mechanism of action
