Overview

Dentin tubule surfaces and the exposed dentin matrix represent a critical structural target in restorative dentistry and the treatment of dentin hypersensitivity (StatPearls, 2023). Dentin is a mineralized tissue composed of hydroxyapatite crystals embedded in a collagenous matrix, permeated by microscopic tubules that connect the pulp to the enamel-dentin junction (NCBI, 2022). When protective layers like enamel or cementum are lost due to erosion or recession, these tubules become exposed to the oral environment. According to the hydrodynamic theory, external stimuli cause fluid movement within the tubules, which stimulates mechanoreceptors in the pulp, resulting in sharp pain (Journal of Conservative Dentistry, 2014). Therapeutic interventions target this site by either occluding the tubule orifices to block fluid flow or by delivering ions to desensitize the underlying nerves (PubMed, 2021). Furthermore, the exposed dentin matrix is the primary substrate for dental adhesives and remineralization strategies aimed at restoring tooth integrity.
