Overview

The dermal-epidermal junction (DEJ) basement membrane zone is a complex multi-layered structure that anchors the epidermis to the underlying dermis (StatPearls, 2023). It consists of several distinct layers, including the hemidesmosomes of basal keratinocytes, the lamina lucida, the lamina densa, and the sublamina densa region containing anchoring fibrils (NIH, 2022). Key molecular components include type XVII collagen (BP180), BP230, laminin-332, and type VII collagen, which work together to maintain skin mechanical integrity and regulate cellular behavior (PubMed, 2021). The DEJ BMZ is the primary site of pathology in various autoimmune blistering diseases, such as bullous pemphigoid, where autoantibodies target these structural proteins, leading to dermal-epidermal separation (Journal of Investigative Dermatology, 2020). It is also the site of genetic defects in epidermolysis bullosa, where mutations in BMZ proteins result in extreme skin fragility (Nature Reviews Disease Primers, 2021). While not a single drug target, the individual proteins within this zone are critical focal points for immunosuppressive therapies, monoclonal antibodies, and emerging gene therapies.
