Dermatan sulfate (DS) is a linear polysaccharide belonging to the glycosaminoglycan (GAG) family, primarily found in the skin, blood vessels, and heart valves (Trowbridge & Gallo, 2002). It is characterized by repeating disaccharide units of N-acetylgalactosamine and L-iduronic acid, which distinguish it from chondroitin sulfate and provide unique protein-binding properties (NCBI, 2023). Biologically, DS plays a pivotal role in regulating blood coagulation by specifically activating heparin cofactor II, a plasma inhibitor that neutralizes thrombin (Monagle et al., 2001). Beyond its antithrombotic activity, it is essential for wound healing, cell signaling, and maintaining the structural integrity of the extracellular matrix through its presence in proteoglycans like decorin (Mizumoto et al., 2015). Pathologically, the accumulation of dermatan sulfate is a hallmark of several mucopolysaccharidoses, while its deficiency is associated with musculocontractural Ehlers-Danlos syndrome (Mizumoto et al., 2017). In the pharmaceutical industry, dermatan sulfate and its derivatives are utilized as anticoagulants that offer a lower risk of bleeding compared to traditional heparin-based therapies (PubMed, 2022).
Dermatan sulfate selectively potentiates the inhibition of thrombin by heparin cofactor II (HCII), a mechanism distinct from heparin which primarily acts via antithrombin III (Monagle et al., 2001). It also functions as a co-receptor for various growth factors, modulating signaling pathways involved in cell growth and tissue repair (Mizumoto et al., 2015). Additionally, it contributes to the structural integrity of the extracellular matrix by interacting with collagen fibers through proteoglycans (Trowbridge & Gallo, 2002).
