Overview

Dermatophagoides pteronyssinus allergen Der p 23 is a major allergen derived from the European house dust mite. It is a small, 7-kDa peritrophin-like protein that functions as a chitin-binding component within the peritrophic matrix of the mite's digestive tract (UniProt P0C1U8). Der p 23 is highly clinically relevant, as it is recognized by IgE in a large majority of mite-allergic patients and is frequently associated with the development of bronchial asthma (PubMed: 23453165). As a therapeutic target, it is a critical component of house dust mite allergen extracts used for allergen-specific immunotherapy (AIT), such as Odactra and Acarizax (FDA). These treatments aim to induce immune tolerance by shifting the patient's immune profile from a Th2-mediated allergic response to a Th1/Treg-mediated regulatory response, which is often monitored by an increase in allergen-specific IgG4 levels (PubMed: 30101436). Successful desensitization to Der p 23 and other mite allergens reduces the inflammatory response and clinical symptoms upon subsequent environmental exposure.
