The descending pain modulatory system (DPMS) is a complex neural network that regulates the transmission of nociceptive information from the periphery to the brain. Centered in the midbrain periaqueductal gray (PAG) and the rostral ventromedial medulla (RVM), this system projects to the spinal cord dorsal horn to either inhibit (antinociception) or facilitate (pro-nociception) pain signals. It utilizes various neurotransmitters, most notably endogenous opioids, serotonin, and norepinephrine, to adjust the gain of the pain experience based on emotional, cognitive, and environmental contexts. In chronic pain conditions such as fibromyalgia or neuropathic pain, the DPMS often becomes dysfunctional, characterized by a loss of inhibitory control or an increase in descending facilitation, contributing to central sensitization. Pharmacological therapies like opioids and antidepressants (SNRIs and TCAs) target this system by enhancing descending inhibition, thereby providing systemic analgesia. Because the DPMS is a multi-component physiological pathway rather than a single molecular target, it serves as a broad framework for multi-modal pain management strategies.
