Dietary free iron in the gastrointestinal lumen refers to the pool of non-heme iron that remains unabsorbed as it passes through the digestive tract (NIH Office of Dietary Supplements, 2023). While iron is a vital micronutrient required for oxygen transport and DNA synthesis, its presence in the lumen can be double-edged (StatPearls, 2023). Unabsorbed iron in the colon can catalyze the formation of reactive oxygen species through the Fenton reaction, leading to mucosal damage and inflammation (PubMed, PMID: 25500555). Furthermore, many enteric pathogens rely on luminal iron for virulence and proliferation, meaning excess iron can shift the gut microbiota toward a dysbiotic state (PubMed, PMID: 31430946). Therapeutic strategies targeting luminal iron include the use of oral chelators or sequestering agents to treat systemic iron overload or to reduce local inflammation in conditions like inflammatory bowel disease. Monitoring this target is crucial for managing patients with hereditary hemochromatosis or those receiving chronic iron supplementation.
