Dietary inorganic phosphate refers to the inorganic phosphate ions (Pi) present in foods—especially as additives in processed foods—which are a critical dietary source of phosphate. Inorganic phosphate is readily absorbed in the intestine, much more so than organic forms present in natural foods. Phosphate is essential for the formation of bones and teeth (as hydroxyapatite), energy metabolism (via ATP), structure of DNA/RNA and cell membranes, cellular signaling, and pH buffering. However, excessive intake—particularly from food additives—can disrupt phosphate and calcium homeostasis, especially in individuals with impaired renal function, leading to complications such as bone disease and increased cardiovascular risk. In clinical practice, phosphate is monitored as a laboratory value, and excess phosphate is reduced using dietary restriction and phosphate binder medications in at-risk populations such as CKD patients.
Phosphate binders reduce intestinal absorption of dietary phosphate by binding to it in the gut. Some drugs act by modifying renal phosphate handling (e.g., by regulating parathyroid hormone or FGF23 signaling).
