Dietary starch and digestible carbohydrates are the principal sources of glucose in the human diet, consisting mainly of the glucose polymers amylose and amylopectin (StatPearls, NBK544242). In the gastrointestinal lumen, these molecules are systematically broken down by salivary and pancreatic alpha-amylase into smaller oligosaccharides, which are further hydrolyzed into monosaccharides by brush-border enzymes such as maltase and sucrase (PubMed, 15846630). Although these carbohydrates are nutrients rather than therapeutic protein targets, they are the substrates for enzymes that are key pharmacological targets in the management of type 2 diabetes. By slowing the rate of carbohydrate digestion, drugs like alpha-glucosidase inhibitors reduce the magnitude of postprandial blood glucose elevations (NIH, MedlinePlus). This therapeutic approach helps in maintaining glycemic control and reducing the risk of long-term diabetic complications. However, the presence of undigested carbohydrates in the distal gut can lead to side effects such as osmotic diarrhea and gas production through bacterial fermentation (PubMed, 15846630).
