Dietary starches and other digestible polysaccharides are complex carbohydrates that serve as the primary source of glucose in the human diet (StatPearls, 2023). In the gastrointestinal lumen, these molecules, primarily amylose and amylopectin, are broken down by salivary and pancreatic alpha-amylase into smaller oligosaccharides (Reactome, R-HSA-189085). These products are further hydrolyzed by brush-border enzymes such as alpha-glucosidase into glucose, which is then absorbed into the bloodstream (PubMed, PMID: 11739013). While these polysaccharides are not therapeutic targets themselves, they are the substrates for enzymes that are targeted to manage metabolic disorders (DrugBank, DB00284). By inhibiting the digestion of these polysaccharides, drugs like acarbose can effectively lower postprandial blood glucose levels in patients with Type 2 diabetes (NIH, 2022). Consequently, the management of these polysaccharides in the gut is a key strategy in treating hyperglycemia and obesity (PubMed, PMID: 25104338). The therapeutic challenge lies in the fact that undigested polysaccharides reach the colon, where they are fermented by microbiota, leading to gastrointestinal side effects (PubMed, PMID: 10448529). Monitoring efficacy typically involves measuring postprandial glucose excursions and long-term glycemic control via HbA1c (StatPearls, 2023).
