Dorsal nerve of the penis (DNP)

The dorsal nerve of the penis (DNP) is the deepest terminal branch of the pudendal nerve, providing the primary sensory (somatic) innervation to the glans and shaft of the penis. It plays a pivotal role in male sexual physiology by serving as the afferent pathway that transmits sensory stimuli to the spinal cord and brain to trigger the erectile reflex and modulate the timing of ejaculation. In clinical pharmacology, the DNP is a target for local anesthetics such as lidocaine and prilocaine, which are applied topically or injected to provide analgesia for surgical procedures like circumcision or to manage premature ejaculation by reducing glans sensitivity. Pathological hypersensitivity of the nerve is associated with primary premature ejaculation, while traumatic or surgical damage can result in neurogenic erectile dysfunction or sensory loss. Therapeutic strategies targeting the DNP include pharmacological blockade, surgical selective neurectomy, and transcutaneous electrical stimulation.
