Overview

The dorsal rami of the thoracolumbar nerves are the posterior branches of the spinal nerves originating from the thoracic and lumbar segments of the spinal cord. They play a dual role in physiology, providing sensory innervation to the skin of the posterior trunk and the zygapophysial (facet) joints, while also supplying motor control to the intrinsic deep muscles of the back, such as the multifidus and erector spinae (StatPearls, 2023). Although they are anatomical structures rather than molecular entities like receptors or enzymes, they are critical focal points in interventional pain management for conditions such as chronic low back pain and facet joint syndrome (PubMed, 2021). Therapeutic interventions often involve the local delivery of sodium channel blockers, such as lidocaine or bupivacaine, to inhibit nociceptive signal transmission. Additionally, glucocorticoids may be applied to these nerves to suppress inflammatory mediators, and neurolytic agents or radiofrequency ablation are used to physically disrupt the nerve fibers for extended analgesia (NIH, 2022).
