Ecological modulation of the gut microbiota represents a therapeutic approach that targets the entire microbial ecosystem within the gastrointestinal tract rather than a specific host molecular receptor or enzyme (Nature, 2022). Drugs and therapies in this category, such as live biotherapeutic products like Rebyota and Vowst, work by reintroducing a diverse array of beneficial microbes to restore ecological balance (FDA, 2023). This restoration helps suppress pathogenic species through competitive exclusion and the production of inhibitory metabolites like short-chain fatty acids and secondary bile acids (Science, 2021). Consequently, this approach is increasingly utilized for treating recurrent infections and is being explored for broader applications in autoimmune and metabolic health (Journal of Clinical Investigation, 2021).
