Overview

Effector T cells (Teff) and alloreactive immune cells represent a broad category of activated lymphocytes rather than a single molecular target. Effector T cells are the progeny of naive T cells that have encountered their cognate antigen and differentiated into cells capable of mounting an immune response, such as CD8+ cytotoxic T cells or various subsets of CD4+ helper T cells (Abbas et al., Cellular and Molecular Immunology). Alloreactive immune cells are specifically defined by their ability to recognize and respond to foreign (allogeneic) major histocompatibility complex (MHC) molecules, a process that is the fundamental cause of graft-versus-host disease (GvHD) and transplant rejection (PubMed: 30241751). Because these cells drive tissue damage in alloimmune and autoimmune contexts, they are the primary focus of immunosuppressive therapy. Pharmacological management typically involves targeting specific molecules expressed by these cells, such as the IL-2 receptor (CD25), calcineurin, or co-stimulatory molecules, to inhibit their activation and proliferation (StatPearls: Graft Versus Host Disease). Consequently, while effector T cells are the therapeutic objective, the actual drug targets are the signaling proteins and receptors that govern their behavior.
